THE PREDICTIVE VALUE OF THE NEWS2 SCORE FOR SEVERITY IN PATIENTS ADMITTED TO THE EMERGENCY DEPARTMENT OF 115 PEOPLE'S HOSPITAL

Abstract

Background: There is a need for early validated of the patient’s severe condition to guide possitive treatment and determine the effectiveness of treatment. Early warning scales are a useful tool to quickly find groups of severe patients. The National Early Warning Score 2 (NEWS2), lauched in 2017 is widely used in emergency department and clinical department, initially showing good value in screening patients with the possibility of serious progression.

Objectives: Determining the severe condition predictive value of NEWS2 score on patients admitted to the Emergency Department – 115 People's Hospital in the first 24 hours.

Methods: Prospective observational analysis study. Study population: Patients admitted to the Emergency Department during the study period. Exclusion criteria: patients <16 years old, cardiac arrest before admission, pregnancy, spinal injury. The ROC curve (Receiver Operating Characteristic) and the AUC (Area under curve) were used to determine the outcome prognostic value. The optimal cut-off point is determined by the Youden index.

Results: The NEWS2 score has good value in predicting outcome with patients entering the Emergency Department in the first 24 hours (AUC = 0.88, 95% CI: 0.85 - 0.88). Sensitivity 92%, specificity 73%, the optimal cut-off point was 7 points. The group of cerebrovascular accident patients onset within 24 hours had NEWS2 score 4 points higher than the other group, 95% CI: 2 – 5 (p<0.05) but there was no difference in mortality (p<0.05).

Conclusions: The NEWS2 score has good value in predicting the condition of severe patients admitted to the Emergency Department within the first 24 hours.
