Abstract

Background: Large scalp defects following tumor resection, trauma, or neurosurgical procedures remain a significant reconstructive challenge. The scalp fasciocutaneous flap offers an optimal solution because of the thickness, robust vascularity, and the ability to preserve hair-bearing tissue.

Objectives: To describe the surgical outcomes of scalp defect reconstruction using local fasciocutaneous flaps at our institution.

Methods: We conducted a case series of 8 patients with large scalp defects who underwent reconstruction with local fasciocutaneous flaps. Clinical characteristics included age, sex, etiology of the defect, size and area of the defect, flap design, postoperative complications, flap survival, cosmetic results, and hair regrowth.

Results: The mean age of patients was 56.6 years (range 27–73), with an equal distribution of 4 males and 4 females. The average defect size was 51.8 cm² (range 16–132.6 cm²). All flaps survived completely, with no cases of infection. At ≥ 6 months follow-up, most patients achieved satisfactory cosmetic outcomes with restoration of hair growth.

Conclusions: Local fasciocutaneous flaps are a safe and reliable option for the reconstruction of large scalp defects, providing durable coverage with favorable cosmetic and functional results.
