Abstract

Background: Influenza virus causes acute respiratory infection with high transmissibility, which can result in epidemics and pandemics. The annual morbidity and mortality burden of influenza remains high. The clinical features of patients infected with influenza A subtype H3 virus have not been well described.

Objectives: To describe the clinical and laboratory features, treatment, and outcome of adult inpatients infected with influenza A/H3 virus.

Methods: A case series report study including all patients ≥16 years old admitted to the Hospital for Tropical Diseases with a diagnosis of influenza A/H3 based on a positive RT–PCR result of nasopharyngeal swabs.

Results: The study included 56 cases, most of whom were > over 65 years old (73.2%) and had underlying conditions (69.6%). Common symptoms were cough, fever, and dyspnea. 83.9% of patients had pneumonia, with chest X-rays showing diffuse interstitial infiltration on both sides of the lungs. 73.2% of patients had respiratory failure, and 14/56 patients progressed to ARDS. All patients were treated with oseltamivir, and 91.1% were treated with antibiotics. The mortality rate was 5.4%.

Conclusion: Influenza A/H3 virus can cause acute respiratory infection with respiratory failure at a relatively high rate, especially in the elderly with underlying medical conditions.
