Abstract

Background: Otitis media with effusion (OME) is a common pediatric condition that affects hearing and language development. Adenoid hypertrophy plays a significant role in the pathogenesis of OME and adenoidectomy offers effective treatment in this group of patients.

Objective: The efficacy of this procedure in OME with adenoid hypertrophy by comparing the data of otoscopy and tympanometry pre and post adenoidectomy

Methods: A prospective case series study. Total 25 patients of otitis media with effusion and adenoid hypertrophy equal to or younger than 15 years old, attending Ear Nose Throat hospital HCMC from August 2020 until June 2021. These patients were followed up with repeated otoscope and tympanometry at 3 months post-surgery.

Results: The common age group between 6 and 10 years old is observed with a 3:1 male to female ratio. Pre-operatively, retraction and amber appearance of the tympanic membrane are frequently encountered in otoscopy (76% and 68% respectively) whereas type B is the dominant finding in tympanometry with 88%. Postoperative 3th months: On otoscopy: normal tympanic membrane: 56% - retraction: 44%, amber coloured tympanic membrane: 38%, normal color tympanic membrane: 32%. Tympanometry showed type B curve in 68% of ears, type As curve: 2%, type A curve: 30%.

Conclusion: Otoscopy with tympanometry provides objective and highly accurate screening tools in diagnosis and monitoring OME with adenoid hypertrophy especially in children. Adenoidectomy is a simple and effective procedure that can be easily performed even in cottage hospitals.
